Highmark Wholecare Provides SDoH Resources for Members

The 2024 Highmark Wholecare Medicaid Member Guide to Social Determinants of Health (SDoH) programs and resources is now available. We believe in providing whole life care for our members and your patients - this care goes beyond standard coverage and provides our members with resources to achieve overall health in all aspects of their life. This guide details the programs we offer to address the various barriers that our members face daily.

These programs take into account the SDoH needs of our members, your patients, by connecting them with:

Access to healthy foods to maintain a nutritious diet.
Affordable housing to keep families together and safe.
Reliable transportation to get members to and from doctor visits and other important destinations.
Education and career counseling to achieve financial independence.
